1. Application of these Terms
These Terms and Conditions govern registration for, and participation in, courses run by NCOSHA U LTD ("NCOSHA"). By submitting this registration form you agree to be bound by them.
2. Registration and Eligibility
Registration is only complete once this form, the required supporting documents (passport photo and academic certificates), and at least the applicable deposit have been submitted and received. NCOSHA may decline or defer a registration where information provided is incomplete, inaccurate, or where a cohort is full.
3. Fees and Payment Plans
The course fee may be paid in full (100%), or as a 70% instalment with the balance settled before the cohort start date, or as a 30% deposit to secure a seat with the balance settled before the cohort start date. Fees are payable via MTN Mobile Money, Airtel Money, card, bank transfer, or cash at the NCOSHA office, as selected on this form.
4. Cancellations, Postponement and Refunds
NCOSHA reserves the right to postpone or reschedule a cohort where enrolment numbers or other circumstances require it; applicants will be notified and offered a place on the next available cohort or a refund of amounts paid, at their choice. A participant who withdraws is subject to NCOSHA's refund policy in force at the time, available on request from the NCOSHA office.
5. Attendance and Assessment
A minimum of 90% attendance is required to qualify for final assessment. Certification requires an overall average assessment score of at least 80%. Learners who do not meet these thresholds may be offered a resit or repeat opportunity at NCOSHA's discretion, which may attract an additional fee.
6. Certificate Issuance
The NCOSHA certificate and safety card are issued once a participant has satisfied the attendance and assessment requirements above and cleared any outstanding balance. Certificates for online-mode participants are shared electronically, with a physical copy available on request. A lost, damaged, or amended certificate may be reissued on written request, subject to a replacement fee.
7. Participant Conduct
Participants are expected to engage respectfully and professionally with trainers, NCOSHA staff, and fellow participants, to complete assessments honestly, and to comply with any health and safety instructions given during practical sessions. Conduct that seriously breaches this expectation may result in suspension or removal from the course without refund.
8. Health, Medical and Emergency Information
Participants must disclose, in the "Health & Emergency Contact" section of this form, any medical condition that may require reasonable accommodation, and must provide accurate emergency contact details. NCOSHA will use this information solely to respond appropriately in the event of an incident during training.
9. Photography and Publicity
Where a participant has given the publicity consent on this form, NCOSHA may use their name, photo and/or testimonial for promotional purposes, including on its website and social media. A participant may withdraw this consent at any time by written notice to NCOSHA.
10. Data Privacy
Personal information collected on this form is processed solely for course administration, certification, quality assurance, communication, and legal or regulatory compliance, in line with the data privacy consent given above. NCOSHA does not share this information with third parties except where required by law, necessary to verify certification with an employer or accrediting body, or with the participant's consent.
11. Force Majeure
NCOSHA is not liable for delay, postponement, or cancellation of training arising from circumstances beyond its reasonable control, including natural disasters, government directives, disease outbreaks, or power/internet outages. Where practicable, affected participants will be notified promptly.
12. Complaints and Amendments
A participant dissatisfied with any aspect of the course, including an assessment result, may raise a written complaint with the NCOSHA office, which will be acknowledged and addressed within a reasonable period. NCOSHA may revise these Terms from time to time; the version in force at the time of a participant's enrolment governs that enrolment, unless a change is required by law.
